Client waiver and consent

Before your first session with FitFix Official, the personal training practice of Omid Ramez, you agree to this waiver in the booking form: you tick “I agree” and type your full name, which counts as your signature. Please read it carefully.

1. Your health

• The health information you give in the booking form and in your assessment is complete and true, and you’ll tell Omid about any change in your health, medication or injuries.
• If you ticked any item in the health screening question, or you’re not sure whether exercise is safe for you, get clearance from a doctor or physiotherapist before you train. Omid may ask for it in writing.
• You’ll stop and tell Omid straight away if you feel pain, dizziness, chest pain, shortness of breath or anything unusual.

2. Screening, not diagnosis

FitFix Official provides personal training and corrective exercise coaching, not medical care. Assessments and tests screen how your body moves; they don’t diagnose anything. Read the health disclaimer.

3. The risks of exercise

Exercise carries risks, including muscle soreness, strains and sprains, joint injury, falls and, rarely, heart problems. Omid adapts every session to you, but no training is free of risk. You take part of your own free will and accept these risks.

4. Release of liability

To the extent the law allows, you release Omid Ramez and FitFix Official from claims for injury, loss or damage that arise from your training, except where caused by their gross negligence.

5. Emergencies

In an emergency, you agree that Omid may call 911, give first aid and contact the emergency contact you named in the booking form.

6. Training at Level Up

Sessions are held at Level Up Health & Wellness, #800 – 801 Marine Dr, North Vancouver. You’ll follow the gym’s rules. Whether Level Up asks you to sign its own waiver: [To be confirmed].

7. Photos and videos on Instagram

• The booking form asks separately whether FitFix Official may share photos or videos of you from your sessions on its Instagram. Saying no doesn’t change your training in any way.
• If you say yes, you can change your mind at any time by emailing Ramez1976omid@gmail.com. We’ll remove the posts we control, but we can’t control copies others may already have saved or shared.

8. Policies

• All sessions are 55 minutes.
• Cancel or reschedule at least 24 hours before a session.
• Full refund if you cancel at least 10 days before your first session. No refunds after that.
• Packages must be used within 12 months of registration.
• GST (5%) applies to all prices.

Your answers are collected by Calendly and shared with Omid. Read how we handle them in our privacy policy.

10. Your agreement

By ticking “I agree” and typing your full name, you confirm that you have read and understood this waiver and are at least 18 years old. Clients under 18: [To be confirmed: parent or guardian signature].
